Early Recognition of Intestinal Obstruction/Severe Constipation/Urinary Retention

Alliance Medical Team emphasizes the importance of early recognition of intestinal obstruction, severe constipation, and urinary retention in patients. We believe that a significant percentage of readmissions are due to system failure to recognize patient health crises, therefore these electronic medical records do not alert staff about the absence of bowel movements or urination. We developed a new monitoring system to track and solve these problems where it addresses this type of readmission dilemma.

Recognition of Fall Risk Patients

Alliance Medical Team neurologists will personally conduct the training sessions for the staff. Nurses receiving this training will understand these neurological conditions as well as provide proper care for patients with deteriorated neurological coordination which includes imbalancement, gait problems and ataxia. This specialized program aims to alleviate potential surprises and issues related to fall risks among neurological patients.

Early Recognition of Sepsis

Alliance Medical Team stresses the importance of early recognition and standardization of the screening process to minimize sepsis-related readmissions. We provide an extensive in-service education, supportive team and 24 hour infectious disease consultants to play a crucial role of reducing sepsis related patient readmissions within the facility.  

Early Recognition of Fluid Overload

We discovered that the majority of Congestive Heart Failure (CHF) patients are at risk of readmission within the first 30 days due to failure to recognize fluid overload and improper diuresis. We generated a new process of identifying vulnerable patients and supporting them with a 24 hour cardiology team before they are readmitted to a medical institution. Alliance Medical Team records have shown that our new process achieved remarkable success for  preventing patients from being submitted into a hospital due to their CHF.

Appropriate Setting for Selected Groups

Alliance Medical Team wants to avoid sending patients back to the hospital for hospice or palliative support because we believe it is a form of futile care that adds more stress onto the grieving family. We provide a 24/7 hospice/palliative care consultation that will allow the patient to live their last days with integrity and dignity with their family in their facility.

Systematic and Standardized Review

Alliance Medical Team focuses on addressing every patient fall, confusion and readmission back to the hospital. Our medical team conducts a systematic and standardized review of the patient’s most recent psychomotor medications where we generate a plan of gradually reducing its dosage to prevent another health risk. We have found that the majority of the patients with confusion and encephalopathy are not mainly due to excessive use of psychomotor medications but from other medical dysfunctions, therefore we provide around the clock psychologist to evaluate and address their mental health crisis.
