Our committed medical providers and staff deliver continuous, around-the-clock care to proactively tackle early signs of dehydration, weight loss, and decreased oral intake in patients, preventing the need for readmission to the hospital. The Alliance Medical Team has compelling evidence that addressing these symptoms at their onset is crucial; neglecting them can lead to a rapid decline in a patient’s health. This oversight significantly increases the risks of severe weakness, incoordination, falls, renal dysfunction, and drug toxicity. By prioritizing early intervention, we can safeguard our patients' well-being and promote healthier outcomes.

Alliance Medical Team neurologists will personally conduct the training sessions for the staff. Nurses receiving this training will understand these neurological conditions as well as provide proper care for patients with deteriorated neurological coordination which includes imbalancement, gait problems and ataxia. This specialized program aims to alleviate potential surprises and issues related to fall risks among neurological patients.

Early Recognition of Fluid Overload

The Alliance Medical Team is committed to effectively tackling critical issues such as patient falls, confusion, and hospital readmissions. Our dedicated professionals conduct a thorough and standardized review of each patient's psychotropic medications, crafting a tailored plan to gradually reduce dosages when necessary, thereby minimizing potential health risks.  Through our extensive experience, we have found that the majority of confusion and encephalopathy cases stem not from the overuse of psychotropic medications, but rather from various underlying medical conditions. To ensure comprehensive care, we provide continuous psychological support, available 24/7, to assess and manage any mental health crises. Our holistic approach empowers patients on their path to recovery and enhanced well-being.

Our research has revealed that a significant number of patients with Congestive Heart Failure (CHF) face a heightened risk of hospital readmission within the first 30 days. This critical challenge often stems from unrecognized fluid overload and ineffective diuresis. In response, we have implemented an innovative process to proactively identify at-risk patients, providing them with dedicated support from a 24-hour cardiology team before they require readmission. The impressive results recorded by the Alliance Medical Team demonstrate that this proactive approach has significantly reduced hospital admissions due to CHF, highlighting our commitment to improving patient care and outcomes.

These essential resources are pivotal in our efforts to dramatically minimize sepsis-related patient readmissions within our facility, ensuring better outcomes and enhanced patient care.

Alliance Medical Team firmly believes in the critical importance of early detection of intestinal obstruction, severe constipation, and urinary retention in patients. We have identified that a significant percentage of readmissions stem from systemic failures in recognizing these health crises. Alarmingly, existing electronic medical records do not provide alerts regarding the absence of bowel movements or urination, which can lead to serious complications.  To tackle this urgent issue, we have developed an innovative monitoring system specifically designed to track these vital indicators. By improving awareness and responsiveness, our system has the potential to greatly reduce preventable readmissions and enhance overall patient care. Together, we can champion a more proactive approach to patient health, ensuring that no one falls through the cracks in our healthcare system.
